We are wanting to put down a concrete side walk in our back yard, from our deck to the door from the garage. We are thinking of using one of the forms for this that have the decorative patterns. Has anyone else done this? What are your thoughts?

I haven't done this and have only seen small areas, like around flower beds. How long is the walk? It might be a bit labor intensive but they look great. Hopefully someone will be by that has done them.

I am talking about the decorative forms that I have seen at my home center, that you put down and then pour in the concrete. I have the advertisements that you can use these for a side walk.

those're the quikcrete forms that you'd use to cast individual 'stones' ??? they're fine imo,,, just be sure to 'set' them right in the ground.

Yes, that is exactly what I am talking about. We want to do something, but a little different and we thought that might me a good choice. Me and dh usually work pretty good together so we thought that we could do that. I will let you know how it turns out.
